This information, besides the company logo, is most often found on the card’s front.
If Rx is available, the front of the card includes the group number as well as Rx carrier information.
The back of the card is reserved for telling hospitals and clinics how to bill insurance. These numbers include the claims mailing address, payer ID (also known as the EDI number), and the numbers for eligibility, preauthorizations, and claims.
